Our Continuum of Care
Street Outreach
We identify persons experiencing homelessness, assess their barriers to accessing services, enroll them in our identification program, and refer them to crucial resources in our community.
Housing & Readiness
Through our Permanent Supportive Housing, Homelessness Prevention, & Rapid Rehousing programs, we assist individuals and families experiencing or at risk of homelessness with multiple forms of aid to secure housing and create stability. We pair the resources with our Rise with Grace Case Management program to ensure long-term success.
Rise with Grace
Our signature case management program, Rise with Grace, is a one-on-one case management program. Clients are paired with a coach to help build relationships and community to break generational cycles of poverty and homelessness.
Love First Project
Our hope for the future, Love First is our solution to help families break the cycle of poverty and homelessness by pairing our Rise with Grace program with affordable housing and community.
